
A dental clinic, a physiotherapy centre or an aesthetic practice in Palma lives by its diary. Every slot left empty by a patient who does not turn up is a professional's paid time with no income, and every reminder call reception makes is time not spent on the person standing at the desk. Automated appointment reminders solve both problems at once, and in Mallorca, where many clinics serve residents of several nationalities and tourists in high season, the message in the right language at the right time makes a noticeable difference to how full the diary is.
What an automated appointment reminder system does
The basic workflow has three pieces: the clinic's diary, a messaging channel and the logic that connects the two. In practice it works like this:
- 24 hours before the appointment, the patient receives a WhatsApp message with the date, time, practitioner and address.
- The message offers two responses: confirm or cancel. There is no need to call.
- If they confirm, the diary is marked as confirmed and reception can see it at a glance.
- If they cancel, the slot is released and the waiting list comes into play (more on this below).
- If they do not reply, a second reminder can be sent a few hours beforehand, or the case can be passed to reception for a call.
We use the official WhatsApp Business API, with approved templates, so messages arrive from a verified number with the clinic's name. For patients who do not use WhatsApp, the same workflow can send an SMS or an email.
The waiting list that fills gaps on its own
This is where the biggest saving lies for many clinics in Palma. When a patient cancels with 24 hours' notice, instead of letting the slot go to waste, the system checks the waiting list and notifies the patients who asked for an earlier appointment or who indicated availability for that time band.
The first to accept gets the slot. The diary is updated, the rest of the list receives a brief message that the slot has now been filled, and reception has not touched the phone.
For this to work well, a few rules need to be defined with the clinic: how many people to notify at once, how long to wait for a reply, and which treatments fit which slot lengths. A dental clean does not fit into the slot of a ten-minute consultation, and the system has to know that.
Preliminary instructions and follow-up afterwards
A well-made reminder is not limited to the time. Depending on the type of appointment, the message can include specific instructions:
- Fasting beforehand for a blood test or a procedure under sedation.
- Bringing previous reports, X-rays or the health card.
- Where to park near the clinic, something that in the centre of Palma saves real delays.
- Consent forms the patient can read calmly at home.
After the visit, the same channel serves for follow-up: a message the day after a treatment asking how they are feeling, a reminder at six months for the check-up, or a notification that the report is now available. All of this is defined by treatment type and triggered from the diary, without anyone having to remember.
Integration with Doctoralia and the diary you already use
Most of the clinics we speak to already have a management tool: Doctoralia, clinic management software, or even a shared Google Calendar in smaller practices. We build on what exists. With Doctoralia, for example, appointments that come in through its portal are read via its integration and trigger the same reminders as appointments taken by phone. With clinic software that has an API, we connect directly; with one that does not, there are periodic export options that also work.
We build the logic bespoke and install it on a server within the European Union, which matters a great deal when handling health data. The result for the clinic is that it keeps working in its usual diary and the messages go out on their own.
Data protection in clinics: what needs to be clear
Health data is a special category under the GDPR and clinics know this well. A reminder system has to respect a few rules by design:
- The reminder message contains the minimum necessary: date, time, practitioner and address. Never the diagnosis or the treatment in plain text.
- The patient has given consent to receive communications through that channel, and this is recorded.
- Data is processed and stored on EU servers, with access limited to those who need it.
- There is a record of which messages were sent, when and with what response, which is also useful for resolving disputes about a cancelled appointment.
- The patient can ask for messages to stop with a simple reply, and that request is honoured automatically.
None of this adds cost or complexity if it is taken into account from the start. What does turn out expensive is setting something up quickly with a free tool that stores data outside Europe and having to rebuild it later.
How much it costs and what to expect
A reminder system with confirmation, waiting list and preliminary instructions is a mid-scope automation. Our published range for an automation is €3,000 to €6,000, depending on the diary you use and how many treatment types need configuring. If the clinic also wants an agent that answers patients' common questions on WhatsApp (opening hours, indicative prices, how to get there) and passes to reception whatever it cannot resolve, that fits within the managed agent, from €500 per month.

Frequently asked questions
Does it work if my patients are foreign residents or tourists?
Yes. The language of the message is chosen according to the patient's record or the language they first wrote in. In Palma clinics with a large international clientele it is common to configure Spanish, Catalan, English and German, and to add others if needed. WhatsApp templates are approved per language.
What happens if a patient replies with something other than confirm or cancel?
Any free-text reply (a question, a request to change the time) is passed to reception with the appointment context so that a person can deal with it. The system does not try to guess. If the clinic has a managed agent, it can resolve the most common questions and pass on the rest.
Do I have to change my management software?
No. We work with the diary you already have, whether Doctoralia, a clinic management program or a Google Workspace or Microsoft 365 calendar. Only if your current tool allows no form of access to the appointments will we tell you so in the assessment, before you spend anything on building.
